▼ Human Cytomegalovirus (HCMV), also known as Human Herpesvirus-5, is a widespread pathogen. In immunocompromised individuals, such as AIDS patients and transplant recipients, as well as in neonates, HCMV infection can lead to severe disease and death. It is the most common congenital infection leading to disabilities such as mental retardation and hearing impairment. The available virostatic drugs are characterized by severe side effects and emergence of resistant strains.
At the cellular level, HCMV can either remain latent or start its lytic replication cycle. Lytic replication is initiated by the expression of immediate early (IE) genes which trigger a cascade of subsequent gene expression programs, eventually leading to viral DNA replication and the production of new virus progeny. While HCMV is able to enter the cell at any cell cycle stage, IE gene expression can only be initiated in G0/G1 phase cells and is blocked in S/G2 phase cells. This block is mediated by the interaction of cellular cyclin A-dependent kinase and the viral tegument protein pp150.
Here, the physiological relevance of cell cycle-dependent IE gene expression for HCMV was addressed by employing a mutant virus lacking the cyclin A-binding motif in the pp150 protein. It was observed that the initiation of IE gene expression in S phase cells results in a stable G2 arrest followed by efficient expression of further viral gene products and viral DNA replication. Only a small subpopulation of cells entered a mitotic arrest and showed no signs of viral DNA replication.
The virus-permissive G2-arrest was dependent on pUL21a. This viral protein has recently been shown to destabilize cyclin A, thereby leading to a cell cycle arrest at the transition from G1 to S phase. An HCMV double-point mutant where both pp150 and pUL21a are disabled in cyclin A-binding showed severe negative consequences for virus and host. A majority of infected cells was forced into mitosis accompanied by a sharp decrease of cell viability. Analysis of the production of new virus progeny revealed an approximately 500-fold growth defect. These were not only additive but synergistic effects of pUL21a and pp150 mutations.
Taken together, it could be shown that the viral proteins pp150 and pUL21a functionally cooperate to prevent HCMV from entering a non-productive mitotic state of infection. These results may point to a model where cyclin A sensing by pp150 alone is not absolutely required for productive HCMV infection but has been developed to strengthen and support cell cycle synchronization by pUL21a.

▼ Background: Carcinoma of the cervix uteri has an incidence of 530,000 per year
and is the second most common carcinoma in women worldwide. More than 270,000
die of it annually. Especially women in countries with no or less developed
screening methods are at risk. An undisputable role in developing cervical
dysplasia or carcinoma plays infection with the sexually transmitted high-
risk(HR)-HPV-types. Therefore, one of the goals of cervical carcinoma
prevention is to obviate HPV-infection and cervical dysplasia or respectively
detection and treatment at an early stage. This thesis deals with 160 patients
tested negative for HPV-infection despite pathologic cytological or
histological clinical evidence. By follow-up examinations of discrepant
results of a genotyping test for human papillomaviruses discrepancy should be
resolved. Methods: Female patients with a Pap-smear of ≥ Pap IIId and/or a CIN
in spite of a negative HPV test were included. Out of 1,254 included samples
160 HPV-negative were screened again by the Multiplex HPV Genotyping Kit for
Research (Multimetrix) in Epidemiology for a total of 24 HPV-types. 57 samples
remained HPV-negative and were therefore sent to laboratories in Jena and
Heidelberg. In Heidelberg, the samples were screened by a multiplexed
analyzing system for 55 HPV-types and the viral load was estimated. In Jena,
the samples were screened for the oncogene E7 for the seven most prevalent HR-
HPV-types and also a GP-5+/6+-PCR-EIA was performed. The corresponding
cytological/histological samples were subsequently sent to three independent
experts for reevaluation of the cytological results. Results: In 102 of 160
HPV-negative samples (63.75 %)
Berlin found HPV with the Multimetrix-Kit,
which were in conclusion defined as false-negative. One sample was excluded
due to lack of material. HPV was found in 13 out of the 57 remaining negative
samples in Jena. 44 remained negative. 28 samples were tested HPV-positive in
Heidelberg, partly with rare HPV-types not included in the Multimetrix-Kit. 26
remained negative; three were also negative for β-Globin (human DNA) and thus
excluded from the screen. All but two Pap-smears were down-graded by at least
one expert. Conclusion: In most cases, discrepancy between positive cytology
or histology and negative HPV results were resolved. 27 of 160 patients (16.9
%) remained HPV-negative. None of these, whose samples were referred to a
cytological reevaluation, had a Pap-smear > Pap IIId. The cytological review
as well as the HPV-testing methods showed some differences when comparing the
results. Thus, further research is necessary to improve screening examinations
for prevention of cervical cancer.

▼ Infectious dermatoses such as ectoparasitoses and dermatomycoses are globally
associated with morbidity, loss of income and social exclusion. Their
distribution in society is inhomogeneous with a disproportionately high burden
in socioeconomically disadvantaged groups, particularly in countries of the
global South. However, little is known about the epidemiological determinants
in these populations. This knowledge is essential for effective control
measures. In order to generate meaningful data for the Amazon region of
Brazil, the inhabitants of Liberdade, a rural district of the small town Coari
were systematically examined for the presence of ectoparasites and
dermatomycoses. Sociodemographic, economic, environmental and behavioral risk
factors were collected using standardized interviews. The diagnoses were made
according to established clinical case definitions. At the time of the study
Liberdade was inhabited by 1123 individuals. 993 individuals were included in
the study (89.8% of the residents of Liberdade); 397 (40%) had at least one
type of dermatosis. Pediculosis capitis was the most common dermatosis
(20.1%). The second most common dermatosis was foot mycosis (17.1%).
Pityriasis versicolor, tinea corporis and tinea capitis had in total a
prevalence of 7.8%. The ectoparasitoses cutaneous larva migrans, scabies and
tungiasis altogether only had a prevalence of 2.1%. Long hair was the most
important independent risk factor for Pediculosis capits (adjusted OR: 5.3
(95% CI: 2.5-11.3)). School age (5-9 years; OR: 9.4 (95% CI: 3.9-22.9)) and
female sex (OR: 2.9 (95% CI: 2.1-4.1)) were also important risk factors. The
prevalence of foot mycoses increased with increasing age (over 60 years: OR:
60.4 (95% CI: 16.1-227.1)); pityriasis versicolor and tinea corporis were most
common among the 15 to 19 years-olds (OR: 3.2 (95% CI: 1.4-7.9)). Sex had no
significant influence on the presence of superficial mycoses. Poverty-
associated factors, such as sharing clothes and towels, absence of waste
collection, unsafe drinking water, crowding, low quality of housing, low
household income and food shortages in the past 12 months, showed a
significant association with the presence of all infectious dermatoses (with
the exception of foot mycoses). It has been shown that infectious dermatoses
in rural Amazonia are an important disease group, with an alarmingly high
prevalence in the population, and thus represent a public health problem. In
addition to age and gender, poverty and poverty-associated characteristics are
the main risk factors. The close link between the prevalence of infectious
dermatoses and poverty indicates that attempts to eliminate this group of
diseases without addressing poverty will not be successful.

▼ Background: Imaging the eye’s retina with optical coherence tomography (OCT)
allows investigating retinal layers. It is now known that the retinal nerve
fiber layer (RNFL) is reduced in patients with multiple sclerosis (MS) in
comparison to healthy subjects. Furthermore, a correlation between RNFL
thickness and brain atrophy assessed by magnetic resonance imaging (MRI) was
shown. Currently, OCT is a potential candidate for monitoring disease
progression in MS. Objective: To further investigate the association between
retinal markers from OCT with brain atrophy markers from MRI. Methods: In four
studies, patients with relapsing-remitting MS or clinically isolated syndrome
were investigated with OCT, MRI and magnetic resonance spectroscopy. Results:
An association between RNFL and brain atrophy was confirmed. Furthermore, RNFL
was associated with neurochemical parameters for neurodegeneration in the
visual cortex. A previous optic neuritis interferes with the link between
brain atrophy and RNFL. Furthermore, in early MS patients a correlation
between RNFL and brain atrophy could only be seen for white matter. Summary:
OCT is a promising candidate for monitoring disease progression in MS
patients. Potential conclusions about general or brain disease progression are
limited by disease duration and optic neuritis.

▼ New strategies in discovery of breast cancer are needed to avoid overtreatment and to cure more aggressive tumors. Our aim was to find an additional diagnostic strategy, which is highly sensitive and specific and helps to evaluate the individual metabolic tumor subtype in order to create personalized therapy concepts.
At Martinos Nuclear Magnetic Resonance Center of the Massachussetts General Hospital we created metabolic profiles of 162 frozen breast tissue specimens by high resolution magic angle spinning proton magnetic resonance spectroscopy (HRMAS 1HMRS). We specifically examined the role of choline metabolites in the Kennedy pathway, namely choline, phosphocholine and glycerophosphocholine. We compared our results with clinical data of each patient. In 50 cases, we semiquantitatively measured choline kinase expression levels, one of the main enzymes within choline metabolism, by Western Blotting.
We demonstrated a significantly higher level of glycerophosphocholine concentration in malignant tissue (p<0,001). Glycerophosphocholine concentration was also positively correlated with younger age (p=0,018) and higher amounts of tumor cells (p=0,003). Patient with a very high level of glycerophosphocholine, however, showed a significantly better survival rate in the univariate analysis (p=0,049). Glycerophosphocholine concentration was significantly lower in big tumors (p=0,023). Choline was significantly higher concentrated in healthy tissues of older patients (p=0,011). Furthermore, choline concentration was significantly higher in HER2-positive tumors compared to HER2-negative tumors (p=0,027). Regarding phosphocholine, we found a significantly better survival rate in cases with higher phosphocholine concentration in our univariate analysis (p=0,048). Phosphocholine is also highly significantly positive correlated with choline concentration (p<0,001). In benign tissues, we discovered a significant negative correlation between the concentration of the enzyme choline kinase and the corresponding phosphocholine level (p=0,005). Our investigations of different molecular tumor subtypes led to promising trends for being able to differentiate these subtypes by their choline metabolites, but we were mostly lacking significant results due to low case numbers.
In conclusion, the detected metabolite concentrations in choline metabolism showed a huge variability. Considering the entire metabolic profile and metabolite ratios may increase the information gained from HRMAS 1HMRS studies rather than evaluating individual metabolite concentrations. HRMAS 1HMRS has the potential to provide a much better understanding of a given tumor’s biochemistry and metabolic make-up. This method in combination with currently existing diagnostic possibilities can give us a better picture of the individual tumor subtype of each patient to develop personalized treatment plans, which improve cancer cure rates and minimize side effects of chemo- and radiotherapy.

Introduction:
Anatomic differences between men and women are important in colorectal cancer surgery. The identification of anatomic structures and the realization of adequate oncologic resection in men appears difficult due to the male narrow pelvis. Several studies consider male gender as a significant risk factor of anastomotic leakage. Surgical complications do not only influence postoperative course but also may delay adjuvant tumour therapy.
There are many studies analyzing the effect of gender on anastomotic leakage. Yet there is a lack of investigations giving an overview on gender differences in colorectal cancer therapy. Hence, primary endpoint of this study was to detect gender differences in therapy and outcome in patients with a colorectal adenocarcinoma in UICC-stages I-III. It was mainly focussed on surgical therapy and its complications.
Methods:
Data collection was performed in a retrospective setting. In this study the data of patients with a colorectal adenocarcinoma of the caecum, colon or rectum in UICC stages I-III, treated with curative intention at the Charité between 2010-2014, were analyzed. Apart from gender other variables influencing therapeutic outcome (age, ASA, BMI….) were collected. Complications were grouped into several subcategories (anastomotic leakage, infection, wound healing disorder…). Postoperative complications were distinguished from complications appearing during course of time. In order to determine the grade of complications, Clavien-Dindo classification was used.
For statistical analysis SPSS versions 22 and 23 were used. Chi-Square-test or Fisher’s exact test were used to determine independence of the nominal variables. Statistical significance was defined by a p-value <0,05.
Results:
The analyzed group consisted of 409 men and 278 women. Male sex was associated with a higher ASA degree. Postoperative complications required a stoma implantation in men more frequently (p=0,029). The rate of surgical complications (bleeding, suture insufficiency…) was higher in men than in women (p=0,025). There were no significant differences between men and women concerning anastomotic leakage. Reinterventions (Clavien-Dindo IIIb) were more frequent in men (p=0,022). Women lived significantly longer than men (p=0,011).
Conclusions :
Men required more frequently reinterventions with secondary stoma implantation, which may be attributed to surgical difficulties during primary tumour resection. Women have a better prognosis, which is reflected in a longer overall survival. This can be led back to the higher comorbidity of the men.

▼ Weltweit leiden ca. 20 % der Bevölkerung an allergischer Rhinitis, die am häufigsten durch den Kontakt mit Pollen hervorgerufen wird. Gegenwärtig leben mehr als 50 % der Weltbevölkerung in Städten. Ungeachtet dessen verfügen selbst große Städte oftmals nur über eine Pollenfalle zur Bereitstellung von Informationen zur Pollenbelastung der Luft. Daher gibt es immer noch ungenügende Informationen über die räumliche Verbreitung von Pollen innerhalb einer Stadt und die möglichen Auswirkungen auf die Gesundheit der Einwohner. Zum besseren Verständnis der Verteilung von Pollen in urbanen Räumen wurde 2014 der Pollenflug der allergenen Arten Birke (Betula), Gräser (Poaceae) und Beifuß (Artemisia) mit einem Messnetz von 14 Pollenfallen in der Großstadt
Berlin sowie der Flug von Birken- und Gräserpollen mit vier Pollenfallen im Großraum
Berlin/Brandenburg gemessen. Darüber hinaus wurden die ermittelten Pollendaten und anonym übermittelte, ortsgebundene Daten zu polleninduzierten Symptomen dazu verwendet, zeitliche und räumliche Beziehungen zwischen diesen beiden Variablen im Großraum
Berlin/Brandenburg, in der Stadt
Berlin und erstmalig auch in Teilbereichen einer Stadt am Beispiel Berlins zu untersuchen. Die zeitlichen Verläufe der Birken- und Gräserpollensaison ergaben für alle möglichen Kombinationen der vierzehn Fallen in
Berlin einen signifikant positiven Zusammenhang. Für Beifuß waren diese Zusammenhänge weniger stark ausgeprägt und nur teilweise signifikant. Entgegen diesen vorwiegend saisonalen Übereinstimmungen traten deutliche Abweichungen hinsichtlich der räumlichen Verteilung der Pollen innerhalb des Stadtgebietes zu Tage, wobei die Unterschiede beim Beifuß am größten waren. Die statistischen Beziehungen zwischen den ermittelten Pollenzahlen und den gemeldeten Symptomstärken während den entsprechenden Pollensaisons waren in der Stadt
Berlin, in deren Teilbereichen und im Großraum
Berlin-Brandenburg größtenteils signifikant positiv, wobei der schwächste Zusammenhang beim Beifuß auftrat. Die Zahl der Birken- und Gräserpollen war in den Randbezirken der Stadt höher als im Stadtzentrum. Damit einher gingen Unterschiede in der Symptomschwere zwischen Stadtrand und Stadtzentrum, wobei die gemeldeten Symptomstärken am Stadtrand im Mittel höher lagen als im Zentrum. Beim Beifuß zeigte sich kein solcher Zusammenhang.
Die vorgelegten und diskutierten Ergebnisse der Studie
- belegen die sehr unterschiedliche Verteilung wichtiger allergener Pollenarten innerhalb einer Großstadt,
- verdeutlichen den Vorteil des Einsatzes mehrerer Pollenfallen für das Monitoring von Pollen in Großstädten,
- unterstreichen die große Bedeutung von Pollen als wichtige Auslöser von allergischer Rhinokonjunktivitis und allergischem Asthma.
Die fortschreitende Urbanisierung verlangt nach einer noch eingehenderen Forschung zur Pollenexposition und Pollenallergie in Städten.

The infant mortality rate (IMR), a key indicator of the quality of a healthcare system, has remained at approximately 3.5‰ for the past 10 years in Germany. Generic quality indicators (QIs), as used in Germany since 2010, greatly help to ensure such a good value but do not seem to be able to further reduce the IMR. The neonatal mortality rate (NMR) contributes to 65-70% of the IMR. The presented approach therefore proposes single-case analysis of neonatal deaths on base of medical records. Most electronic medical records still contain large amounts of free-text data. Semantic evaluation of such data requires the data to be encoded with sufficient classifications or transformed into a knowledge-based database.
Methods
The Nordic-Baltic classification (NBC) was used to detect avoidable neonatal deaths. This classification has been applied to a sample of 1,968 neonatal death records, which represent over 90% of all neonatal deaths in East
Berlin from 1973 to 1989. All cases were analyzed as to their preventability based on the complete perinatal and clinical data by a special commission of different experts.
The developed approach allows databases accessible via SQL (Structured Query Language) to be searched directly through semantic queries without the need for further transformations. Therefore, I) an extension to SQL named Ontology-SQL (O-SQL) that allows to use semantic expressions, II) a framework that uses a standard terminology server to annotate free-text containing database tables and III) a parser that rewrites O-SQL to SQL, so that such queries can be passed to the database server, have been developed.
Results
The NBC was used to select the group of cases that had a high potential of avoidance. The selected group represented 6.0% of all cases, and 60.4% of the cases within that group were judged avoidable or conditionally avoidable. The automatic detection of malformations showed an F1 score of 0.94. Furthermore, the generability has been proved with different semantic queries and was measured with
between 0.91 and 0.98.
Conclusion
The results show, that the presented method can be applied automatically and is a powerful and highly specific tool for selecting potentially avoidable neonatal deaths and thus for supporting efficient single case analysis. The seamless connection of ontologies and standard technologies from the database field represents an important constituent of unstructured data analysis. The developed technology can be readily applied to current data and supports the increasingly important field of translational research.

studies show a high rate of detected bacteria in presumed aseptic revisions (4-15%) (3;4;5;6;7). A single positive culture does not necessarily lead to the diagnosis of a periprosthetic joint infection (PPI) and can cause difficulties in interpretation (8).
Sonication is able to detect bacteria from the surface of explanted endoprosthesis and reaches the highest sensitivity and a high specificity out of culture-based microbiological methods to detect PPI (8;9).
The aim of this study was to evaluate the influence of singular positive sonication cultures on the implant survival and the functional and radiological outcome after revision arthroplasty. A retrospective cohort study was performed, and included a clinical and radiological examination of patients who underwent one-stage hip revision arthroplasty due to presumed aseptic loosening. Based on the results during the revision arthroplasty (synovial fluid culture, histopathological examination of periprosthetic tissue, periprosthetic tissue cultures, sonicate fluid cultures), we divided the patients into three groups: patients with PPI (PJI:
n=13); patients without detection of bacteria in sonication (AS:
n=13); patients with detection of bacteria in sonication (SFC:
n=13). After an average follow-up of 34.20 month (minimum follow-up 12 months) a clinical (Harris-Hip-Score, WOMAC-Index, SF-36, VAS) and radiological (detection of periprosthetic osteolysis) examination was performed. We found no significant difference in the rate of loosening, pain intensity (VAS), radiological outcome and for the results of disease-specific functional scores (Harris-Hip-Score and WOMAC-Index). Only three out of eight domains in the SF-36 shows significant difference, while we found none of them in the total score and the two summary scores. In these domains (“physical functioning“ p=0,026; “social role functioning“ p=0,01; “mental health“ p=0,021), SFC achieves significantly poorer results than one of the comparison group, without significant differencesbetween the comparison groups.
In conclusion, during the investigation period, there was no increased rate of reoperations or a poorer functional and radiological outcome for patients with singular positive sonication cultures during hip revision arthroplasty. Only 3 domains of the SF-36 showed significant differences between the groups, however without a conclusive connection. Significant better results of the aseptic cohort compared to both supposedly septic groups did not result. The validity of this study is reduced by the sample size (
n=47) and the length of the follow-up. Further studies should be performed -with a greater sample size and designed as a longitudinal study- in order to answer the leading question.

▼ Introduction: Polypectomy of colorectal polyps is a well-established method to prevent colorectal cancer. However, the risk of recurrences and malignancy increases with the size of the polyp. Recent studies on lesions ≥ 2 cm have shown that despite high recurrence rates after polypectomy, recurrences can be treated endoscopically in > 90 % of cases with strict colonoscopic surveillance. The aim of the current study is to evaluate the efficacy of endoscopic resection of colorectal polyps ≥ 3 cm (“Giant lesions”) in a tertiary referral centre.
Method: From January 2006 to June 2014 in 212 patients (mean age 67,7 years, 50,9 % women, 49,1 % men), 224 colorectal polyps ≥ 3 cm were endoscopically resected. In a retrospective analysis the recurrence and endoscopic success rate had been determined. The data had been collected by the clinic documentation system and over 150 referring specialists via standardised questionnaires.
Results: The mean polyp size of the 224 colorectal polyps was 4,0 ± 1,2 cm. 88,4 % were sessile/flat (
n = 198). 93,8 % of the polypectomies were performed using a piecemeal technique (
n = 210). In 24,1 % of the colorectal adenomas (
n = 54) a high-grade intraepithelial neoplasia (HGIEN; 18,8 %,
n = 42) or invasive carcinoma (5,4 %,
n = 12) was evident. In 42,4 % (
n = 95/224) of the polypectomies a recurrence occurred in a median follow-up period of 18 months (1−96 months) and on average two surveillance colonoscopies (1,97 ± 1,14). A residual adenoma was present in 39,3 % (
n = 88/224) and a recurrence after one negative colonoscopy in 10,7 % (
n = 6/56) of the cases. Overall, 95,6 % (
n = 194/203) of the colorectal polyps could be successfully treated endoscopically in 1,49 ± 0,82 sessions and surgery could be avoided. Independent risk factors for recurrence were increasing polyp size (p = 0,002), sessile/flat morphology (p = 0,003) and the endoscopist (p = 0,001).
Conclusion: Polypectomy is an effective treatment of colorectal polyps ≥ 3 cm. Due to the high recurrence rate, a strict colonoscopic surveillance is indispensable for the success of the treatment.

▼ Einleitung: Aufgrund verbesserter hochauflösender Ultraschall-Geräte können viele Organauffälligkeiten bereits im Ersttrimester-Screening detektiert werden. Ziel dieser Studie war es, das Potential des Ersttrimester-Screenings (ETS) hinsichtlich der Detektionsrate für fetale Fehlbildungen im Vergleich zum Zweittrimester-Screening zu evaluieren. Sekundäre Fragestellung war die Erfassung der Häufigkeit isolierter und in Kombination mit Fehlbildungen auftretender sonografischer Softmarker.
Methodik: Im Zeitraum von August 2010 bis September 2013 wurde eine prospektive Studie in einer auf Feindiagnostik spezialisierten Praxis von DEGUM II-Experten in
Berlin zur Detektion von Fehlbildungen im Erst- und Zweittrimester-Screening durchgeführt. Eingeschlossen wurden alle Feten bei denen ein Erst- und Zweittrimester-Screening durchgeführt wurde. Der Befund des Ersttrimester-Screenings wurde mit demjenigen des Zweittrimester-Screenings und dem postnatalen Schwangerschaftsausgang verglichen.
Ergebnisse: Es wurden insgesamt 10294 Feten erfasst, von denen 235 sonoanatomische Auffälligkeiten und 598 isolierte sonografische Softmarker aufwiesen. 115 Feten hatten einen Fehlbildungsbefund im Ersttrimester-Screening. Nach Komplettierung der Untersuchung im Zweittrimester-Screening zeigte sich bei 120 im Ersttrimester-Screening unauffälligen Feten ein Fehlbildungsbefund. Im Zweittrimester-Screening zeigten sich bei 173 Feten Fehlbildungsbefunde. Es resultierte eine Sensitivitä
t von 48,1%, eine Spezifitä
t von 99,96%, ein positiv prädiktiver Wert von 96,5% und ein negativ prädiktiver Wert von 98,82% für die Detektion von Fehlbildungen im Ersttrimester-Screening. Die Sensitivitäten variierten stark je nach betroffenem Organsystem und der jeweiligen Diagnose und erreichten die höchsten Werte für das fetale Abdomen (84%), die fetale Wirbelsäule (67%), das fetale Gesicht (57%) und das fetale Skelett (56%).
142 Feten aus dem Fehlbildungskollektiv und damit 7,2% der gesamten Studienpopulation wiesen sonografische Softmarker auf. Der intrakardiale echogene Fokus stellte den häufigsten Softmarker in dieser Studie dar. Bei 60,4% der Feten mit Fehlbildungen konnten zusätzlich sonografische Marker nachgewiesen werden. Fast drei Viertel der isolierten sonografischen Marker im zweiten Trimester waren singuläre Befunde. Das hypoplastische Nasenbein war der häufigste Softmarker bei Feten mit Fehlbildungen im ETS.
Schlussfolgerung: Im Ersttrimester-Screening konnte mit positiv prädiktiven Werten von mehr als 89% fast die Hälfte der Fehlbildungen detektiert werden.
Sonografische Marker sind häufig und kamen bei 7,2% der gesamten Studienpopulation vor. Vor allem singuläre isolierte sonografische Marker sind meist temporäre Befunde, die nach unauffälligem Ersttrimester-Screening und nach Ausschluss struktureller Anomalien nicht zur Verunsicherung werdender Eltern führen sollten.

▼ The hepatocellular carcinoma is the second most frequent cancer related cause of death worldwide. It is usually a complication of a cirrhosis of the liver or of other risk factors such as a chronical hepatitis B infection or the non-alcoholic fatty liver disease. Being often diagnosed only at a late stage, mortality is very high (15 % 5-year-survival-rate). Besides the common staging systems, no further prognostic or predictive markers for an individually adapted treatment have been established. Angiogenic factors are potentially non-invasive markers for (the progression of) HCC and cirrhosis and at the same time possible therapy objectives. The retrospective study at hand investigated a cohort of 263 HCC patients and the influence of clinical and paraclinical parameters on their survival. We determined and compared three factors of angiogenesis (PlGF, VEGF and sVEGFR-1) within the HCC cohort (
n=263), a cohort of cirrhosis patients (
n=82) and a cohort of healthy probands (
n=30).
The overall median survival amounted to 33 months. Transplanted patients had the best out-come: more than 60% were still alive after 20 years of observation. End-stage patients had a median survival of 4.5 months. The univariate analysis showed that higher age, alcohol-related liver disease, number and size of lesions, higher tumor stage (with respect to BCLC, CLIP, Okuda and TMN staging systems) as well as higher values of AFP, VEGF and sVEGFR-1 were connected statistically significantly to shorter survival. Patients treated aggressively during intermediary stage survived statistically significantly longer. VEGF had an approximately in-dependent influence on survival. Healthy probands had higher PlGF, VEGF and sVEGFR-1 values than HCC- and cirrhosis patients (mean values in pg/mL: for PLGF: healthy: 9,68/ cirrhosis: 3,48/ HCC: 5,26; for VEGF: healthy: 300,36/ cirrhosis: 241,57/ HCC: 224,37; for sVEGFR-1: healthy: 189,81/ cirrhosis: 60,27/ HCC: 122,32). PlGF values were overall very low. sVEGFR-1 distinguished significantly between HCC and cirrhosis patients (AUC: 0.78, p<0.0001). AFP performed better (sensitivity: 73.8%, specificity: 90.2%). sVEGFR-1 values significantly increased with higher tumor stage, VEGF with higher cirrhosis stage. The correlation between the markers was not strong.
Our study indicated a prognostic value of VEGF and sVEGFR-1. However, we could not con-firm any further benefit for cirrhosis or HCC diagnosis particularely compared to and combi-ned with AFP. They may rather be used as individual and non-invasive follow-up parameters in addition to known clinical and paraclinical markers.

▼ Aktuelle Forschungsergebnisse zur Multiplen Sklerose (MS) und dem dazugehörigen Tiermodell, Experimentelle autoimmune Enzephalomyelitis (EAE), unterstreichen die Rolle von Hirnschranken für den Beginn und die Aufrechterhaltung der Neuroinflammation. In den letzten Jahren rückte die Suche nach Visualisierungsmethoden beeinträchtigter Schranken als Methode zur frühen Diagnostik und zum Verständnis grundlegender pathophysiologischer Prozesse der Neuroinflammation in den Fokus. Gängige Bildgebungsmethoden, die Gadolinium-basierte Kontrastmittel benutzen, haben verschiedene Nachteile, da sie weder alle Aspekte dysfunktionaler Blut-Hirn-Schranken zeigen, noch eindeutige Klarheit über potentielle toxische Effekte wiederholter Kontrastmittelgaben besteht. Unsere Arbeitsgruppe hat sich in den letzten Jahren der Erforschung neuartiger Magnetresonanz (MR) - Bildgebungsmethoden gewidmet, die es erlauben, in vivo Untersuchungen mit histopathologischen und molekularbiologischen Einsichten zu vereinen. In der vorliegenden Arbeit nutzten wir eine neue Art von elektromagnetischen Nanopartikeln, die Europium-dotierten sehr kleinen Eisenoxid Partikel (engl. Europium-doped very small iron oxide particles = Eu-VSOP), um Störungen der Blut-Liquor-Schranke am Plexus choroideus (CP) zu visualisieren. Dafür führten wir, nach intravenöser Gabe von Eu-VSOP, zu verschiedenen Zeitpunkten der EAE MR-Aufnahmen durch und analysierten fluoreszenzmikroskopisch die dazugehörigen histopathologischen Schnitte. Unsere Ergebnisse zeigen, dass Eu-VSOP die in vivo Visualisierung von Hirnschrankenstörungen und neuroinflammatorischen Prozessen ermöglichen und erlauben, diese post mortem mit Veränderungen des CP zu verknüpfen.
Unsere Forschungsgruppe hat in vorhergehenden Studien mit MR-Elastographie (MRE) in MS und EAE gezeigt, dass chronisch-entzündliche Erkrankungen des zentralen Nervensystems (ZNS) mit einem Verlust der Steifheit des Hirngewebes einhergehen. Im zweiten Teil der vorliegenden Studie führten wir nun MRE-Messungen in EAE-Mäusen durch, um mittels Veränderungen mechanischer Eigenschaften des Hirngewebes Störungen der Blut-Hirn-Schranke (BHS) zu demaskieren. Wir konnten zeigen, dass jene Hirnregionen, die stärker von der Neuroinflammation betroffen sind, besonders starke Verluste der Steifigkeit präsentieren und dass diese Veränderungen, anders als bei Verwendung gadoliniumhaltiger Kontrastmittel, mit dem klinischen Verlauf korrelieren. Weiterhin konnten wir beweisen, dass perivaskuläre Läsionen, in denen die BHS gestört ist, ein Remodeling mit Aggregaten des Extrazellulärmatrix-Proteins Fibronektin (Fn) durchlaufen, und dass die Genexpression dieses Proteoglykans mit MRE-Veränderungen korreliert. Damit stellt diese Studie eine Grundlage zur Visualisierung von Hirnschrankenstörungen als zentraler pathophysiologischer Prozess zur Entstehung neuroinflammatorischer Läsionen sowie zur Implementierung neuer MR-basierter Bildgebungsmethoden als Diagnosemittel der MS dar.

Out of hospital cardiac arrest (OHCA) is one of the most common causes of death worldwide. The rapid and effective application of cardiopulmonary resuscitation (CPR) by bystanders plays a decisive role to improve OHCA outcome. Therefore, it is crucial to implement systematic CPR trainings for lay bystanders, specifically amongst schoolchildren. The
Berlin youth red-cross has been conducting CPR trainings amongst 3rd-6th graders since the 1990s. The effectiveness of these trainings has yet to be formally evaluated. This study set out to assess the short- and long-term effects of a one-time CPR training amongst 6th graders. Furthermore, this study examined whether schoolchildren with prior training developed better skills and knowledge than children without prior training. Methods All courses for 6th graders within a 4-month period were analyzed. The trainings lasted 180 minutes each and covered: emergency call/wound care (90 minutes), unconsciousness/recovery position (70), and cardiac arrest/CPR (20). Knowledge was tested via multiple choice and open-ended questions, skills were assessed by standardized scenario tests. Assessments were performed immediately before (t0), immediately after (t1), and 10 weeks post training (t2). Schoolchildren were asked to provide information about further trainings. Results Overall, 232 schoolchildren from 10 classes participated in the study. Written test: The percentage of correct answers in the written test significantly differed depending upon the time point: t0 30.4%, t1 51.9%, and t2 48.1% (p<0.05). For the 25% of all schoolchildren had partaken in prior training sessions (w/o CPR) no statistical difference could be shown as compared to schoolchildren without prior trainings (t0 35.7% vs 29.7%, t1 57.1% vs 63.1% p=0.57). Results concerning emergency call were very good (t0 91.4%, t1 98.9%). Errors occurred frequently regarding diagnosis of (recovery position t1 80,5% vs respiratory check t1 18.4%) and interventions for cardiac arrest (recovery position t1 10.4%; CPR t1 17.4%). Scenario test: In the practical test post training the children performed adequate chest compressions (t1 91.7%); about one third of the children did not apply a correct recovery position (t1 65.0%) Discussion The training showed an overall long-term positive effect of CPR training amongst participating schoolchildren. Prior training did not show any influence. This study thereby confirms that 6th graders can successfully apply adequate chest compressions. Conclusion The training was effective in teaching 6th graders correct chest compressions. Deficits were seen concerning the identification of cardiac arrest and the appropriate actions required. Teaching the recovery position should be re-evaluated.

▼ Introduction: Every third person in Germany is affected by one kind of child abuse. Since the pioneering work of the Adverse Childhood Experiences Study, it has been shown that early life stress (ELS) effects the individual into adulthood and promotes the development of somatic and mental illnesses. Recent studies suggest an association between early trauma and type 2 diabetes mellitus (DM2), which may be independent of obesity and psychological comorbidities. Aim of this study is to investigate whether there is a change in glucose metabolism and a higher prevalence for risk factors of the metabolic syndrome in somatically and mentally healthy women with ELS.
Material and Methods: A total of 21 women with ELS and 36 women without trauma were enrolled in the study. All subjects were interviewed for traumatic experiences and mental illnesses. Followed by an oral glucose tolerance test (oGTT), examination of laboratory parameters and physical examination for the risk factors of the metabolic syndrome. Data was analyzed using Statistical Package for Social Sciences (SPSS) version 23. Concentration of Glucose and insulin during oGTT was analyzed by repeated measures of variance analysis. An alpha level of .05 was chosen. For the analysis of correlations the area under the curve for glucose and insulin concentration in oGTT was calculated and correlated with severity of childhood trauma and prevalence of metabolic risk factors. Sociodemographic and clinical data were calculated using the
t-test for independent samples and the chi-square test for categorical data.
Results: Subjects with ELS had a significantly higher BMI than the controls. In the oGTT, however, there was no significant difference for the concentrations of glucose and insulin. Even after controlling for BMI, this result did not change. Women with ELS were significantly more likely to have elevated triglycerides, while there was no significant difference between the groups for other risk factors of the metabolic syndrome. There was no significant correlation between severity of the trauma and concentration of glucose and insulin in the oGTT. BMI was positively correlated with the sum in the Childhood Trauma Questionnaire, but further diagnostic criteria for the metabolic syndrome showed no positive correlation.
Conclusion: Somatically and psychologically healthy women with ELS showed no evidence of impaired glucose tolerance or higher prevalence of the metabolic syndrome. It can be assumed that there is no relationship between ELS and DM2 and the metabolic syndrome, which is independent from weight and psychological comorbidities.

T- und B-Zellen ausgeübt. Die Aufrechterhaltung der Gedächtniszellen ist wichtig um dem Organismus einen dauerhaften Schutz zu verleihen. Weiterhin ist eine strategische Positionierung dieser Gedächtniszellen im gesamten Organismus entscheidend um eine zeitnahe Reaktion gegen wiederkehrende antigenische Stimulierungen sicherzustellen. Während die Gewebeverteilung und -erhaltung von Gedächtnis
T-Zellen und Plasma Zellen bereits gut beschrieben wurde, wurde der Lebensstil von Gedächtnis-B-Zellen (Bmem) bislang nicht weiter untersucht. In meiner Doktorarbeit habe ich die Gewebeorganisation und den Lebensstil von Gedächtnis B-Zellen in Mäusen untersucht, welche als Ausgangspunkt für weitere translationale Studien am Menschen dient. Um die Gewebeverteilung zu untersuchen, wurden Isotyp-veränderte Bmem der Milz, des Knochenmarks, von peripherem Blut und von Lymphknoten unter verschiedenen Immunisierungs- und Infektionsprotokollen ausgezählt. Die Mehrheit der Isotyp-veränderten Bmem wurden in der Milz lokalisiert, es ist aber auch eine signifikante Population im Knochenmark enthalten. Der Vergleich des Repertoires des B-Zell-Rezeptors (BCR), einer einzigartigen Bezeichnung jeder individuellen B-Zelle, des Repertoires der Isotyp-veränderten Bmem der Milz und des Knochenmarks hat gezeigt, dass B-Zellen mit derselben BCR (Klonotypen), welche während einer spezifischen Immunreaktion generiert wurden, sich kaum überlappen. Die Mehrheit der Bmem Klonotypen wird ausschließlich in einem der beiden Organe exprimiert, was beweist, dass Isotyp-veränderte Bmem zweier Organe verschiedene Populationen mit nur minimalen Austausch über den Blutkreislauf repräsentieren. Phänotypisch, Isotyp-veränderte Bmem der beiden Organe unterscheiden sich in der Oberflächenproteinexpression von CD21 (Komplementrezeptor) und CD62L (L-Selektin) mit Untergruppen von CD21low und CD21high Populationen im Knochenmark, aber nicht in der Milz. Weiterhin beinhalten Isotyp-veränderte Bmem des Knochenmarks einen höheren Spiegel von CD62L im Vergleich zur Milz. Isotyp-veränderte Bmem des Knochenmarks und der Milz ruhen im G0 des Zellzyklus, wie durch die Expression des proliferativen Markers Ki67 bestimmt wurde, und sind widerstandsfähig gegen in vivo Behandlungen mit Cyclophosphamid (einem DNA alkylierenden Wirkstoff, der wuchernde Zellen tötet). Im Knochenmark befinden sich Isotyp-veränderte Bmem in unmittelbarer Nähe zu retikulären Stromazellen, die VCAM-1 exprimieren. ▼ Überlebende von chirurgischen und konservativen Intensivstationen haben ein hohes Risiko für lang anhaltende kognitive Defizite. Die Sepsis gilt als häufigste und schwerwiegendste der kritischen Erkrankungen, die zu kognitiven Defiziten führen können. Einer von drei Überlebenden einer Sepsis wird mit schwerer, neu aufgetretener kognitiver Dysfunktion aus
dem Krankenhaus entlassen. Chronische Neuroinflammation, diffusive zerebrale Schädigungen und neuronaler Zelltod werden als die primären Korrelate in der Entwicklung lang anhaltender kognitiver Defizite angesehen. Neuere Studien deuten darauf hin, dass
Sepsis eine inflammatorische Aktivierung von Mikroglia auslöst, die zu Phagozytose gestresster, aber funktionsfähiger Neurone führt.
Um ein Mausmodell der Sepsis zu etablieren, wurden Lipopolysaccharide (LPS) in C57BL/6-Mäuse und homozygote Knockout- und Wildtyp-Mäuse mit Mertk-, CD11b- und
Mfge8-Defizienz in einer Dosierung von 1,5mg/kg injiziert. Das akute Krankheitsverhalten und langzeitige kognitive Funktionen der Tiere wurden analysiert, um die Effekte von phagozytotischer Defizienz und Behandlung mit Peptiden auf kognitive Defizite zu untersuchen. Um die Anforderungen an das Tierwohl durch Reduzierung von Handling, Wasser- und/oder Nahrungsentzug und unnötigem Leid optimal zu erfüllen, untersuchten wir
außerdem die Anwendbarkeit eines vollständig automatisierten 8-Arm-Radial Arm Mazes
und von machine learning-basierter Bestimmung von Abbruchkriterien (humane endpoints).
Tiere mit LPS-Injektion zeigten (a) Charakteristika von Krankheitsverhalten unmittelbar nach der Injektion und (b) kognitive Defizite nach der einmonatigen Erholungsperiode. Tiere mit Mertk-, CD11b- und Mfge8-Defizienz präsentierten bessere Lernleistungen bezüglich Place Learning, Place Reversal und Avoidance Conditioning. Behandlungseffekte von Cilengitid oder cRGD konnten bezüglich Sucrose Preference, Avoidance Conditioning und in der frühen Phase des Place Learning beobachtet werden. Bei der Nutzung von Machine Learning-Modellen, die Abbruchkriterien bestimmten, zeigte sich, dass Mäuse im Schlaganfall- und im Sepsismodell mit einem höheren Todesrisiko mit hoher Genauigkeit erkannt werden konnten. Mäuse bis zum Alter von 18 Monaten zeigten effizientes räumliches Lernen im Paradigma für das Arbeitsgedächtnis und im kombinierten Paradigma für das Arbeits- und Referenzgedächtnis im automatisierten 8-Arm-Radial Arm
Maze ohne Nahrungs- oder Wasserentzug.
In einem Mausmodell der Sepsis beobachteten wir eine Verminderung langzeitiger
kognitiver Defizite in phagozyten-defizienten und in Cilengitid- oder cRDG-behandelten
Tieren, was eine Erklärung für die Mechanismen, die langzeitigen kognitiven Defiziten zugrunde liegen, bieten könnte. Minimiertes Leid für die Tiere und verbesserte Reproduzierbarkeit von experimentellen Ergebnissen waren möglich durch die Benutzung machine learning-basierter Bestimmung von Abbruchkriterien und automatisierter
Verhaltenstestung.

▼ Introduction: Severe muscle injury can result in an incomplete healing whereby the contractile fibers are replaced by fat and scar tissue. Current therapeutic approaches aim to support the regenerative capacity of skeletal muscle through the delivery of regenerative cells. In particular, cell-based therapies with mesenchymal stromal cells (MSC) already showed improved muscle function. However, the effects of MSC on the histological level are still unclear, as there are hardly any standardized methods available for analysis. The aim of this work is to investigate the influence of MSC-based therapy on two histological parameters — the total muscle cell density, and the differentiation of cells with centrally located nuclei (regenerated cells) — with a semi-automated algorithm (CSV) which was developed for the presented work.
Methods: The CSV developed to record the two histological parameters was first compared to manual assessment using five histomorphological criteria. In the experimental part, 36 female Sprague-Dawley rats underwent a standardized crush injury in the left soleus muscle. Immediately after traumatization of the muscle, the intervention group (
n = 18) was transplanted with an alginate gel with 1x 106 autologous MSC, while the control group (
n = 18) received only the alginate gel. After 7, 28, and 56 days (
n = 6, respectively), the muscles were harvested for histological analysis. The previously tested CSV was then used to deter- mine the total muscle cell density and the density of regenerated cells.
Results: The results indicated no significant difference between the manual detection and computer-assisted procedure of histological parameters. Using the CSV, a significant increase in total muscle cell density after trauma was observed. In both left, injured and right, uninjured muscles of the MSC-treated group, a significant increase in total muscle cell density was observed at 28 and 56 days, compared to the control group. Regenerated cell density of injured muscles in both groups increased significantly up to eight weeks after trauma compared to uninjured muscles. In the control group, a strong increase in the percentage of regenerated cells was observed on day 28. The MSC-treated muscles, however, showed a gradual increase in regenerated cell fraction along the entire recovery course.
Conclusion: The CSV is a suitable method for recording the total muscle cell density and regenerate cell density. MSC increase the total muscle cell density in injured muscles, possibly explaining the increase in muscle strength. The simultaneous increase in cell density in uninjured muscles indicates a systemic effect of the MSC. Under MSC influence, a prolonged regeneration can be observed.

▼ Introduction: Today, allogeneic stem cell transplantation is still regarded as the only curative
therapeutic option for hematologic malignancies like non-de novo acute myeloid leukemia and
myelodysplastic syndromes. However, data regarding the long-term outcome of allogeneic stem
cell transplantation in these diseases remain limited. The present study examines the long-term
outcome of allogeneic stem cell transplantation in the above-mentioned conditions and reviews
the prognostic und predictive influence of different clinical parameters.
Patients and methods: Data of 268 patients, transplanted at the Department of Hematology,
Oncology and Tumorimmunology of Charité – Universitätsmedizin
Berlin from 1995 to 2015,
were retrospectively analyzed. The Kaplan-Meier-method was applied for the assessment of
overall survival and disease free survival. Relapse and non-relapse mortality rates were examined
using the cumulative incidence-method. Univariate and multivariate analyses were performed
using COX regression models.
Results: Five years after transplantation, the outcome of the entire cohort is: OS: 42.8%,
DFS: 41.0%, KI-Rez: 33.2%, and KI-NRM: 27,2%. Ten years after transplantation, the outcome
is: OS: 35.3%, DFS: 33.2%, KI-Rez: 35.5%, and KI-NRM: 32.6%. The following clinical
characteristics are associated with a significantly reduced OS (p < 0.05): older patient age
(RROS = 1.78), patients diagnosed with sAML or tAML as compared to MDS (RROS = 2.12 and
1.86), RIC compared to MAC (RROS = 2.13), monosomal karyotype (RROS = 2.24), a high
DRI (RROS = 2.51), a hematopoietic cell transplantation-specific comorbidity
index (HCT-CI) > 3 (RROS = 2.08) and a bone marrow blast count of > 20% at the time of
transplantation (RROS = 2.20). The occurrence of acute or chronic GvHD after
transplantation (RROS = 0.66 and 0.33) and the preemptive/prophylactic administration of
DLI (RROS = 0.50) were associated with a significantly higher OS (p < 0.05).
Discussion: With an OS of 35% at 10 years after transplantation, alloTx shows good overall
results. However, successful therapy significantly depends on several factors like age, cytogenetics
and comorbidities. Therefore, detailed risk assessment before treatment is required. By using dose
reduced conditioning regimes, alloTx became an option for elderly patients with clinically relevant
comorbidities. Especially cGvHD after alloTx is associated with an elevated OS, which is most
likely due to the protective graft-versus-leukemia effect. Therefore, a promising approach for
further improving survival might be the preemptive/prophylactic administration of DLI in
appropriate patients.

▼ Introduction: Epithelial ovarian cancer (EOC) is associated with the highest mortality rate among all gynecological cancer. Poor prognosis is especially attributed to recurrent disease, which a majority of women develop, but which to date is only poorly understood. To make matters worse, EOC is known for a high intratumoral genetic variability that impedes an adequate therapeutic management. Apart from that, the anti-tumoral immune response is perceived as a pivotal prognostic factor and a potential base for immunotherapy. Still, research on the heterogeneity of intratumoral immunoinfiltration remains scant. In this study we analyze the dynamic of the intratumoral immune response during progression of high-grade serous ovarian cancer (HGSOC) and therefore compare paired tumor samples of primary and recurrent disease for the first time.
Methods: We systematically analyzed the densities of CD3+, CD4+, CD8+ tumorinfiltrating lymphocytes (TILs) and MHC1, 2 expression by immunohistochemistry on tissue microarrays (TMAs) in 113 paired samples of primary and recurrent HGSOC. TILs quantifying was performed using a new automated software (CD3 Quantifier) for the first time. As part of this study the software was validated and further developed by comparative analyses. All patients had been included to the EU-funded OCTIPS FP7 project.
Results: Our analysis revealed a correlation of TILs density and MHC expression between primary and recurrent HGSOC. The anti-tumoral immune response remained stable during tumor progression, however, part of the tumors showed an enhanced immunogenicity in recurrent tumors. Most of the immunological markers lost their prognostic impact in recurrent disease.
Conclusion: We are the first ones to show a homogenous intratumoral immune response across primary and recurrent tumors in HGSOC. A higher anti-tumoral immune response was not associated with a better prognosis in recurrent tumor. Thus, we assume an immune response during tumor progression might not necessarily be effective. Our findings suggest that tissue samples of a primary lesion could also be used for the immunological analysis of the recurrent tumor. This would allow to gain further insights into the investigation of the so far poorly understood recurrent EOC.

▼ Every year more than 20 000 people in Germany contract rectal cancer. In many cases, after a confirmed diagnosis, a surgical procedure with several days of hospitalization and a lengthy convalescence phase takes place. Despite the most modern medical possibilities, there is a considerable risk of complications. In the 1990s, Professor Kehlet and his team developed a multimodal, interdisciplinary therapy concept. The so-called fast-track-surgery allows a perioperative management which improves the structural process and leads to a reduction of complications and an accelerated recovery. Methods: It is shown how the integration of such a therapy concept into the daily clinical routine has an effect on the patient's well-being or the postoperative outcome. For this purpose, 118 patients undergoing elective rectal resection in the transitional period (2009-2011) after the introduction of the -fast-track-surgery and laparoscopy were retrospectively examined. For further evaluation, preoperative risk markers and postoperative complications, both general and surgical, were recorded. The patient population were divided into two groups (Laparoscopically operated and conventionally operated patients). The evaluation was done comparatively between the groups as well as comparatively between the different years The aim of the work is to show that after the introduction of a "fast-track" treatment concept, a reduction of complications in the field of rectal surgery can be achieved. Furthermore, the differences between laparoscopic interventions and conventional interventions are investigated. Results: The introduction of a fast-track-therapy concept and laparoscopy shows a reduction of complications and an accelerated convalescence in the comparison of the two groups. This results in an improved outcome for the patient through the introduction of a multimodal therapy concept. However, the combined effect of fast-track and laparoscopic interventions as well as possible selection bias of the comparison groups are problematic.

▼ The rotator cuff is responsible for maintaining the function of the shoulder joint. Lesions
of the rotator cuff tendons can be treated operatively using different techniques. The
treatment of accompanying pathologies, such as lesions of the long head of the biceps
tendon, is possible during arthroscopic rotator cuff repairs. Different techniques for the
treatment of lesions of the long head of the biceps tendon are described in literature,
one technique described as lasso-loop-tenodesis by Lafosse.
Massive rotator cuff lesions without the possibility of reconstruction can result in a
painful limitation of movement of the shoulder (cuff tear arthropathy). For these cases,
the reverse shoulder prosthesis was developed. The design of the prosthesis results
in a stabilization by medialization and distalization of the shoulder’s center of rotation.
Thus, the deltoid muscle is partly able to replace the function of the rotator cuff.
Since the invention of the reverse shoulder prosthesis, indications have been
extended. Beside of the cuff tear arthropathy, reverse shoulder prostheses are used in
complex proximal humeral fractures in elderly patients, fracture sequelae and revision
arthroplasty. The surgeon has to consider the long-term results as well as predictive
factors for the functional outcome, since a loss of function in the long-term follow up is
described in literature.
The effect of physical therapy on the shoulder function in the long-term follow up has
not been evaluated.
The aim of the study was to evaluate the functional outcome after lasso-loop-tenodesis
of the long head of the biceps tendon (publication 1), the evaluation of the functional
outcome after reverse shoulder arthroplasty in cases of fracture sequelae and the
identification of prognostic factors (publication 2) as well as the evaluation of physical
therapy in the long-term follow up after reverse shoulder arthroplasty (publication 3).

Behandlungsoption, die jedoch aufgrund fehlender Spenderorgane begrenzt ist. Eine Antwort auf einen Herzinfarkt ist die Aktivierung von Epikardzellen, die sich letztendlich zur Herzlinie im Myokard hin differenzieren können. Injizierung von Vorläuferzellen in den Infarktbereich zeigten in der Klinik widersprüchlichen Erfolg. Im Gegensatz dazu ermöglichen Epikard-Patches mit polymerem oder biologischem Hintergrund eine kontrollierte lokale Behandlung, um die Regeneration des Herzens zu stärken.
Dieses Projekt hatte die Entwicklung eines epikardialen Komposite-Patches zur Unterstützung des kardialen Regenerationsprozesses zum Ziel. Es besteht aus humaner extrazellulärer Herzmatrix (hECM), die eine spezifische Bioaktivitä
t und eine optimale Zellumgebung bietet, und einem polymeren oder amniotischen Gerüst mit basaler Biokompatibilitä
t, die die erforderliche mechanische Stabilitä
t liefert.
Die hECM wurde zu einem Hydrogel (hgECM) verarbeitet, und Gerüste wurden mit hgECM
beschichtet. Die grundlegende Morphologie und die mechanischen Eigenschaften wurden
durch einachsige Ziehversuche und Elektronenmikroskopie gemessen. Die Biokompatibilitä
t wurde durch Adhäsionskapazitä
t, LDH-Freisetzung, BrdU-Einbau und LIVE/DEAD-Färbung bewertet und die Immunogenitä
t wurde durch Zytokinsekretion, Makrophagenpolarisation und
T-Zellproliferation bestimmt. Darüber hinaus wurden transdifferenzierte induzierte Kardiomyozyten-Vorläufer (iCMP) auf Gerüsten zur zellulären Bioverbesserung kolonialisiert. Eine hgECM-Beschichtung bewirkte eine stabile Nanobeschichtung auf den Gerüsten und die mechanischen Eigenschaften wurden nicht beeinflusst. Nach der hgECM-Beschichtung verbesserte sich die Adhäsion bei mehreren Herzzellen und iCMP deutlich. Darüber hinaus induzierte hgECM eine verringerte Zellnekrose und verbesserte Zellproliferation, bestimmt durch LDH-Freisetzung und BrdU-Einbau. Durch LIVE/DEAD-Färbung wurden auf hgECMbeschichteten
Gerüsten mehr Zellen als lebendig indentifiziert. Eine hgECM-Beschichtung
verursachte eine Verringerung der proinflammatorischen Zytokinsekretion bei LPS-aktivierten Monozyten auf beiden Gerüsten und auch von naiven Monozyten auf polymeren Gerüsten. In Bezug auf die Proliferation von
T-Zellen und die Polarisation von Makrophagen wurde keine Aktivierung und keine Beeinflussung durch hgECM-Beschichtung beobachtet.
Zusätzlich wurde ein in vitro Test basierend auf der ersten Reaktion des Immunsystems
etabliert: Polymorph Nuclear Cells. Die Amnionmembran als „Proof-of-Principle“ -Implantat
zeigte in diesem Test keine Immunreaktion, und eine Translation auf jedes Material ist möglich.
Wir haben hier ein neues zusammengesetztes Epikard-Gerüst mit humaner Herz-ECM
entwickelt, das das Potenzial hat, in Großtierstudien und schließlich in klinischen Studien

▼ Background: Thanks to new effective treatments and therefore less severe symptoms more women with rheumatoid arthritis (RA) are able to conceive. Disease management for the mother with the highest safety for the unborn is the most important therapy target. Through analysing the data from the German biologics-register RABBIT and additional data from phone interviews the pregnancies and pregnancy related complications with biological-therapy were compared with the conventional synthetic (cs)DMARD-therapy. Special emphasis was put on evaluating the disease activity and etiopathology during time of conception, pregnancy, postpartum and the breastfeeding period depending on the anti-rheumatic treatment.
Methods: To assess the risk for pregnancy associated complications for RA-patients treated with biologicals compared to those treated with csDMARD-therapy all reports of pregnancies or discontinuation of treatment due to wanting to conceive to RABBIT from May 2001 till October 2012 were examined. The first or following pregnancies of 71 women were reported by their rheumatologist. Data from RABBIT and additionally from the phone interviews were used. The patients were divided into 4 groups according to medication exposure at the time of conception. Results: A total of 55 patients participated in the interview (=77% response). All the registered pregnancies
n=76 have been used. During the observation two patients with rituximab-infusions had an elective pregnancy termination. The result presentation references 74 pregnancies with outcome birth or miscarriage. 10 out the 74 pregnancies in 9 women ended in miscarriage. With 16.7% the biological-therapy exposed pregnancies were most affected. Out of the 64 live-births, 8 were prematurities. The mothers with prematurities subjectively and objectively had a higher disease activity during their pregnancy. With a continued systemic glucocortocoid therapy of 10 mg a day, 100% of births were premature. On average the physician reported disease activity reached 7.2 among prematurities and 2.8 among term deliveries. All newborns were delivered healthy and without congenital malformations.
Summary: The data do not give any indication that therapy with TNF alpha inhibitors is associated with an increased risk of malformations or other adverse pregnancy outcomes. Long term effects are not known yet. Exposure to TNF-alpha inhibitors - especially adalimumab, etanercept, infliximab and certolizumab - can be assumed safe until the first positive pregnancy test. Data concerning rituximab, abatacept, anakinra and tocilizumab are limited and their use during conception, pregnancy and breastfeeding cannot be assessed certainly. Summarized, it is necessary to conduct more studies and longterm observations to provide evidence-based informations on this topic.

▼ Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common genetic disorders, affecting 12.5 million people worldwide. The disease causes the growth of large cysts on both kidneys, replacing the healthy kidney tissue. Polycystic kidneys can reach a weight of several kilograms and due to their size and abnormal anatomy, can cause a variety of symptoms. The symptoms can include chronic pain or recurrent infections that are often difficult to treat conservatively. Besides drug treatment, the nephrectomy has been proven as a surgical therapy option for symptomatic patients.
Successive displacement of the kidney parenchyma by cysts can eventually lead to chronic kidney failure, which typically occurs when the patient is in their fifties. Ultimately, patients are dependent on either dialysis or a renal transplant, hence there are two major surgeries to treat ADPKD – the native nephrectomy and kidney transplantation. This thesis discusses the timing of the native nephrectomy in relation to the date of transplantation.
Purpose: The purpose of the present thesis is to evaluate the postoperative complications following the removal of polycystic kidneys and to compare pre-transplant and post-transplant nephrectomies in order to determine the ideal date of nephrectomy in ADPKD patients. Methods: The retrospective analysis contains the data of 121 nephrectomies, which were carried out between January 2005 and December 2018 on ADPKD patients at the clinic Charité Mitte. The postoperative complications were assessed with the help of the Clavien-Dindo classification; contributing factors were assessed using univariate and multivariate analysis.
Results: All nephrectomies were unilaterally performed via open retroperitoneal access. The nephrectomy was carried out prior to transplantation in 89 cases, and post transplantation in 32 cases. There was no significant difference in terms of postoperative morbidity or mortality between the groups. In general, severe complications were rare and could easily be controlled. The duration of hospitalization was significantly shorter in the post-transplant group while the kidney weight was significantly higher in the pre-transplant group.
Discussion: There is currently a medical consensus that a nephrectomy in ADPKD patients should be carried out only in the presence of severe symptoms and a precise indication. In the present examination, nephrectomy proves to be a safe procedure both for pre-transplant as well as post-transplant patients, however the determination of the ideal timing requires further analysis. No significant differences regarding postoperative complications between the two groups were detected.

▼ We introduce a user-friendly, standardized protocol for tractography of the
major language fiber bundles of the perisylvian language network.
The introduced method uses dMRI (diffusion magnetic resonance imaging)
images whereas the region of interest (ROI)-definition is based on structural T1
magnetization-prepared radio-frequency pulses and rapid gradient-echo (MPRAGE) MRI
templates, without a spatial normalization to Montreal Neurological Institute (MNI) space.
ROI’s for five language relevant fiber bundles were visualized on an axial, coronal or
sagittal view of T1 MPRAGE images. The ROI’s were defined based upon the tracts’
obligatory pathways, derived from literature and own experiences in peritumoral
tractography.
The resulting guideline was evaluated for each fiber bundle in 10 healthy
subjects and 10 patients by one expert and three test persons.
Overall 300 ROI’s were evaluated and compared. The targeted language fiber bundles
could be tracked in 88% of the ROI pairs, based on the raters’ result blinded ROI
placements. The evaluation indicated that the precision of the ROI’s didn’
t relate to the
varying experience of the raters.
Our guideline introduces a standardized language tractography method
for routine preoperative work up and for research contexts. The ROI placement guideline
based on easy to identify anatomical landmarks proved to be user-friendly and accurate,
also in inexperienced test persons.

▼ INTRODUCTION: The allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a standard form of therapy used in acute leukaemia and other hematological neoplasia. Important complications are graft-versus-host-disease (GVHD) and veno-occlusive disease/sinusoidal obstruction syndrome (VOD/SOS). Both diseases are marked by a multifactorial damage of endothelial cells that leads to endothelial inflammatory reactions and thus organ damage. The polyclonal antibody anti-thymocyte-globulin-Fresenius (ATG-F) is used as prophylaxis against GVHD, while Defibrotide (DF) is used as prophylaxis against VOD/SOS. The effects of ATG-F and DF on endothelial cells have yet not been experimentally tested to a sufficient extent. In this work the hypothesis that ATG-F can inhibit endothelial functions and thus impair neovascularization was tested. Secondly, the hypothesis that DF can stimulate endothelial functions and thus promote endothelial regeneration was tested.
METHODS: Three in vitro assays were used to test the effects of ATG-F and DF on human umbilical vein endothelial cells (HUVECS): 1) the scratch assay measures endothelial proliferation and migration; 2) the MTT assay measures endothelial proliferation rates; and 3) the tube formation assay measures the angiogenic activity of RESULTS: ATG-F can significantly inhibit endothelial proliferation and migration in the scratch assay and can dose-dependently inhibit proliferation rates in the MTT assay. ATG-F showed no significant effects on the angiogenic activity in the tube formation assay. DF can significantly inhibit proliferation rates in the MTT assay and the angiogenic activity in the tube formation assay, both dose-dependently. DF showed no effects on endothelial proliferation and migration in the scratch assay.
CONCLUSION: Our findings demonstrate previously unknown effects of ATG-F and DF, two standard drugs used in the field of allo-HSCT, on endothelial cells. Both tested drugs show inhibitory effects on the function of endothelial cells in a dose-dependent manner. These findings may have implications for therapies targeting the endothelium. In the early phases after allo-HSCT, both ATG-F and DF could have a positive effect by inhibiting the pathological GVHD-associated neovascularization, and thus leading to a reduced severity of acute GVHD. However, in later phases after allo-HSCT, the inhibitory effects of ATG-F and DF on endothelial functions may impair endothelial regeneration and thus promote endothelial damage. This might lead to in an increase of endothelium-associated complications after allo-HSCT, such as microangiopathy, chronic GVHD, and VOD/SOS.

Protease-aktivierter Rezeptor 2 (PAR-2) ist ein G Protein gekoppelter Rezeptor (GPCR), der bei Angiogenese eine wichtige Rolle spielt. Autoantikörper, die gegen bestimmte Membranrezeptoren gerichtet sind, können Angiogenese und auch die Entwicklung von Krebs nach Transplantation beeinflussen. Das Vorkommen von Autoantikörpern gegen PAR-2 bei Nierentransplantationspatienten (KTx) und deren potentiell proangiogene Funktion wurden bislang noch nicht untersucht.
Methoden:
Mittels HiTrap Protein G Säulen wurde IgG aus dem Serum von gesunden Kontrollpersonen und nierentransplanierten Patienten isoliert und PAR-2 Autoantikörpertiter durch ELISA bestimmt. In Wildtyp und PAR-2 überexprimierenden HMEC-1 wurden PAR-2 abhängige Signalwege nach Aktivierung durch Trypsin, PAR-2 aktivierendes Peptid (PAR-2 AP) und PAR-2+-IgG wurden mittels Westernblots und Luziferase Reporter Assays untersucht. Real time PCR und ELISA wurden verwendet, um die Effekte der PAR-2 Agonisten und IgG auf die VEGF Freisetzung zu bestimmen. Der Einfluss von PAR-2+-IgG auf die Angiogenese wurde durch Matrigel- Angiogeneseassays mit Endothelzellen analysiert.
Ergebnisse:
PAR-2 Autoantikörpertiter von nierentransplantierten (PAR-2 – IgG) Patienten waren niedriger als in gesunden Kontrollen. In PAR-2 überexprimierenden HMEC-1 wurde durch Trypsin und PAR-2 AP der Gα12/13-, Gq11- und ERK1/2-Signalweg konzentrationsabhängig aktiviert. Dies führte zu einer erhöhten Promotoraktiviä
t, mRNA- und Proteinspiegel von VEGFA. PAR-2+-IgG aktivierte den Gα12/13-, Gq11- und ERK1/2-Signalweg signifikant stärker als PAR-2 – IgG. PAR-2+-IgG erhöhte VEGFA sowohl auf mRNA, als auch auf Proteinebe, welches partiell durch PAR-2 spezifische Blocker gehemmt werden konnte. Angiogenesisassays zeigten, dass PAR-2 – IgG zu einer verminderten endothelialen Netzwerkbildung führt.
Schlussfolgerung:
Die Ergebnisse dieser Arbeit bestätigen, dass PAR-2 an mehrere G Proteine koppelt, darunter Gα12/13 und Gq11, den ERK-Signalweg aktiviert und zu erhöhtem VEGFA in Endothelzellen durch natürlichen Aktivator Trypsin führt. PAR-2 Autoantikörper von gesunden Personen aktivierten die gleichen Signalkaskaden wie Trypsin und PAR-2-AP und waren proangiogen. Dagegen wirkten PAR-2 – IgG von KTx Patienten antiangiogen. Die klinische Bedeutung dieser verminderten in vitro Angiogenese von PAR-2 – IgG von KTx Patienten sollte in zukünftigen größeren Kohortenstudien untersucht werden. Es wäre interessant, inwieweit niedrige PAR-2-Antikörperspiegel z.B. analog zu PAR-1 Autoantikörperspiegeln als Biomarker für ein erhöhtes Krebsrisiko eingesetzt werden könnten.

The regeneration of large bone defects after trauma, tumor resection, or infection is one
of the remaining challenges in orthopedic surgery. Currently, 3D added manufacturing of
Titan-Mesh-Scaffolds is under clinical evaluation as an alternative to autologous bone
grafting. Although it is known that the mechanical environment influences bone
regeneration, it remains unknown if the mechanical properties of rigid Titan-Mesh-
Scaffolds can be tuned to faster bone regeneration. In this study, we investigated the
potential of tuning Titanium-Mesh-Scaffold stiffness to promote bone regeneration in a
large bone defect in a sheep model by optimizing the scaffolds rigidity. We used finite
element techniques to design two Titan-Mesh-Scaffolds that, while having the same
geometry, presented with significant differences in Titan-Mesh-Scaffold stiffness. 3D
added manufacturing was employed to create two types of customized Titan-Mesh-
Scaffolds (i.e., a soft and a 3.5 times stiffer Titan-Mesh-Scaffold), which were filled with
cancellous bone graft and implanted in a 4 cm mid-diaphyseal tibia defect in twelve
sheep. At 24 weeks, radiological and histological analysis showed that an asymmetrical
callus formation that dominated the lateral defect bridging, reflecting the strain patterns
predicted for both Titan-Mesh-Scaffold groups. Bone grew on the outer Titan-Mesh-
Scaffold surface and directly through the inside mesh structure of the scaffold. The soft
Titan-Mesh-Scaffold showed increased endochondral bone formation and earlier
radiological bone bridging, as compared to the stiffer Titan-Mesh-Scaffold. This study is
the first to illustrate the potential of mechanically optimized Titan-Mesh-Scaffolds to
enhance bone regeneration in large bone defects.

▼ Aims: The aims of this study were to test the accuracy of Cameriere et al.’s European formula on a sample of North German children based on dental age (DA) for chronological age (CA) assessment and to adapt the formula used, in case of regional peculiarities of this group of children.
Materials and Method: Unselected orthopantomograms of 1000 children (444 males and 556 females) aged 5-16 years were used. The roots of seven left mandibular teeth (except for 3rd molar) were evaluated. The number of teeth with complete root development (N0) was counted. Teeth with incomplete root development were examined and the distance between the inner sides of the open apex was measured and normalized by dividing it by the tooth length to avoid magnification error.
Results: The results of the regression analysis showed that sex (g), the sum of the width of the normalized open apices (s), number of teeth with closed apices (N0) and the first-order interaction between the normalized apex width of the canine (X3) and N0 contributed significantly to the fit. All previously mentioned factors were included in the regression model, yielding to the following formula:
DA = 9.829 + 0.632N0 - 1.037s + 0.686g - 1.582 (N0 × X3)
where g is a variable: 1 for males and 0 for females.
Cameriere et al.’s European formula underestimated the mean CA of boys by 0.56 ± 1.04 years and of girls by -0.32 ± 0.96.
Conclusion: Camerieres European formula underestimated the chronological age of most age groups in both sexes. The adapted formula explained 84.1% of the total deviance, with a median age of 0.070 years and 1.185 years interquartile range (IQR), and is better for Dental age estimation of North German children.
