Abstract Abstract: Background: The incidence of Human Immunodeficiency Virus (HIV) infection has been increasing between adolescents and young adults. Such persons may present severe emotional, social and health impairments. The objective of this study was to evaluate the progression of psychosocial outcomes in youth with a recent diagnosis of HIV infection (YRDHIV) in the first follow-up year. Methods: Observational, analytical longitudinal cohort study. Thirty-four YRDHIV (85.3% male, aged 21.37 ± 2.5 years), being followed up at a reference center, were compared to a control group (CG) of 30 college students (53.3% female, aged 21.35 ± years). Diferences between personality characteristics (NEO-FFI revised abbreviated personality inventory), quality of life (QoL - WHOQOL-bref instrument), satisfaction with life (Satisfaction with Life Scale - SLS), resilience (Wagnild Young resilience scale), family support (Family Support Perception Inventory) and mental health (Mini International Neuropsychiatric Interview) were evaluated at baseline, 6 and 12 months afterwards. Statistical analysis was performed with Student's "t" test, ANOVA and risk estimation by Odds Ratio (OR). Differences were considered statistically significant if "p" ? 0.05. Results: In the evaluation of personality, the comparison of NEO-FFI-R domain scores showed statistically significant differences at baseline, with lower scores for YRDHIV in Agreeableness and Extroversion and higher in Neuroticism. After 12 months, no differences were observadas between groups. Regarding quality of life, lower scores were observed for all domains in the YRDHIV group at baseline. Such differences were not observed at 6 and 12 months for the psychological, social, total QoL and general perception of health. At 12 months, no differences were observed between groups for the general perception of QoL. Regarding SLS scores, significant differences were observed at baseline and 6 months, with lower scores for the YRDHIV group. At 12 months, there were no differences. No differences were observed between YRDHIV and CG in resilience scores, at any point in time. In the YRDHIV group, family support perception scores were lower for the affective-consistent, family adaptation and total, at baseline. Such differences were not observed at 6 and 12 months of follow-up. Regarding longitudinal evaluation of psychiatric disorders, significantly higher risks for major depressive episode, suicide ideation, agoraphobia and alcohol and illicit substances abuse/dependency were observed for YRDHIV group at baseline. At 6 months, suicide ideation and substance abuse risks were higher for YRDHIV. Agoraphobia and substance abuse risks wher higher for YRDHIV also at 12 months. Conclusion: YRDHIV showed personality, QoL, satisfaction with life, family support perception and mental health markers lower than those observed in a healthy control group.
